Quality Assurance data to be available for OEM engineers and

Any variance in the QA data that exceeds OEM recommendation
will be reported immediately to OEM Engineers by a service call. |

Any variance in image quality will be shown to Radiologist.
Radiologist will have the final word on whether the CT/MRI system
is "up" or "down". |

Monthly
|
|
1.
|
OEM will do preventive maintenance per OEM preventive maintenance
guidelines. |
|
2.
|
Preventive maintenance will be documented and left on the
CT/MR system at the time of service. |
|
3.
|
Any documentation not left on the CT/MR system will be considered
as not being accomplished. |
|
Semi Annual Review
|
|
1.
|
Base films will be gathered during the first week of initial
service. |
|
2.
|
These base films will serve as guideline for all future visits
to CT/MR unit. |
|
3.
|
Client and Radiologist will agree on the Image quality on
the base images. |
|
4.
|
Shared Imaging will have internal Applications Specialist
review base films semi-annually, or more frequently, if a perceived
image quality concern develops. |
|
5.
|
Applications Specialist will pull random films from previous
six months and compare to initial base films. |
|
6.
|
Radiologist and Applications Specialist will review films
together and discuss the quality of images. |
|
7.
|
If image quality has been reduced from the initial base films,
system will be brought back to initial specifications. Radiologist
will have the final word on the quality of the system. |
|
Other
|
|
1.
|
Client will have access to any data that has been acquired
for the purpose of documenting the Quality Assurance process.
This may include access to the daily QA numbers for the CT/MR
system as well as the preventive maintenance documentation.
